Health Information Management is responsible for the storage and upkeep of the medical portion of patient information. Medical records are a key aspect of patient care because they are used to provide continuity of care, for insurance reimbursement process, and to aid both the hospital and physicians in legal cases. But most importantly medical records provide documentation of the care that the patient received while in the hospital.
You can request medical records from Southwest Regional Medical Center in the following ways:

You may visit the Health Information Management Department to fill out the authorization form.
- Your doctor can request your records.
The doctor’s office can call 937-378-7760 to request your medical records. We will fax the requested records directly to the clinic the day before or the day of the appointment. This is the quickest way for your doctor to receive your medical records.

Typically processing time to request medical records is 7-10 business days, depending on the availability of the medical record.
To protect confidentiality of the medical record, information cannot be given over the phone, and the record cannot be copied and released without the written consent of the patient, power of attorney or legal guardian. Court orders are required for records of patients with the diagnosis of mental illness, chemical dependency or AIDS when requested by parties other than the patient.
